Selecting a MTM, or made to measure shirt, follows the two step process the shirt is named after. First, the MTM shirt is made and hung on the rack of a tailor's shop.

MTM shirts are select by customers based on what fits their neck and wrists. Then we move on to measure. The tailor takes some basic measurements of the customer wearing the shirt and uses those measurements to adjust the shirt to make as tight a fit as possible.

Bespoke or custom shirts cut out the already made aspect of MTM shirts. With bespoke shirts, everything begins when you first visit a tailor. The Pros & Cons of MTM Shirts V. Bespoke Shirts
The major advantage of MTM shirts is that they are faster and easier to make. Since the bulk of the shirt is already made and won't need major adjustments, you will be wearing it a lot sooner than a bespoke shirt. MTM shirts are also cheaper than bespoke shirts.

Bespoke shirts come with several advantages MTM shirts just can't match. No minor adjustment made for a MTM shirt will ever compare to how well-fitted a bespoke shirt can be. With bespoke shirts, you also have almost infinite customizability, from what fabric is used to make the shirt to the design of the collar and the pattern of the weave.
